("us" or "our") is offering the ePayments service ("Service") as a convenience option to you, our valued customer ("you" or "your"). The ePayments Service is limited to the ability to pay your Patient Accounts electronically over the Internet. In addition to these Terms and Conditions, you should review our ePayments Service Frequently Asked Questions (FAQ) and Privacy Policy. The FAQ page includes important information about what Internet browsers are currently supported by the ePayments Service, how information sent or received using the ePayments Service is encrypted for security purposes, and other information about how the ePayments Service works and how to get help if you have a problem using the ePayments Service.
Authentication and Authorization for ePayments Service Usage
This ePayments Service incorporates commercially reasonable methods to authenticate the
information that you have supplied for making a payment through the ePayments Service. You will not be able to submit a payment until the ePayments Service completes the authentication process, and you will not be allowed to use the ePayments Service until you have read these Terms and Conditions and have acknowledged doing so by clicking the "I Accept" button. As the user of the ePayments Service, and upon your acceptance of these Terms and Conditions, you will have submitted your banking account or credit card account (your "Designated Account") information. You are responsible for any legal, regulatory, or banking penalties and fees that may be assessed for supplying false information to us for use with the ePayments Service. By accepting these Terms and Conditions, you authorize Ellis Medicine to initiate a charge to your Designated Account according to your instructions. Each time you use the ePayments Service to pay your Ellis Medicine account, you will be reauthorizing us to charge your Designated Account. In addition, by accepting these Terms and Conditions, you represent and warrant that
You are 18 years old or older;
You are using your actual identity and any information you provide is accurate and complete;
You are legally authorized to make payments using your Designated Account; and
Your use of the ePayments Service will not violate any local, state, national or international laws or regulations.
Service
By providing
with the account information for your Designated
Account, you authorize us to follow the payment instructions we receive from you
through the ePayments Service. When you use the ePayments Service to send us a payment instruction, you
authorize
to initiate a message to your bank or credit card provider
to charge your Designated Account and to send those funds to use for payment on your
account so that we receive the funds as close as reasonably
possible from the date indicated by you in the payment instruction. Payment instructions received through the ePayments Service after 3:00 pm ET on a normal business day, will be processed as if it was received on the following day.
By accepting these Terms and Conditions, you agree to use the ePayments Service as it is designed
to make payments to

Any other use of the ePayments Service other than its original design is prohibited.
LIMITATIONS OF LIABILITY AND EXCLUSIONS OF WARRANTIES Alterations and Amendments
These Terms and Conditions are the legal binding agreement between Ellis Medicine and you with respect to the ePayments Service. You will be agreeing to the current provisions of these Terms and Conditions each time you make a payment using the ePayments Service. These Terms and Conditions may have been altered or amended from the last time you used the ePayments Service so be sure to read it each time before you make a payment to be certain you still agree with its provisions. Your use of the ePayments Service after any changes to these Terms and Conditions will constitute your agreement to such change(s).
Termination or Discontinuation
We reserve the right to terminate the ePayments Service at any time, or to terminate your use of the ePayments Service. If we terminate your use of the ePayments Service, you will need to pay your Ellis Medicine account in another manner.
Disputes
If there is a dispute regarding the ePayments Service, you agree to resolve the dispute in accordance with the dispute resolution procedures and governing law as outlined in your Credit Agreement.
These Terms and Conditions are the complete and exclusive statement of the agreement regarding the ePayments Service between you and Ellis Medicine. It supersedes any proposal or prior agreement, oral or written, and any other communications between you and Ellis Medicine relating to the ePayments Service. If there is a conflict between what a Ellis Medicine employee tells you and the terms of this document, these Terms and Conditions will prevail.
You should print and retain this document for safekeeping and reference at any time during usage of the Service.
